Home healthcare services are available for individuals who meet
specific criteria and have a physician's referral.
Eligibility Criteria
- Anyone who is ill
- Recently discharged from the hospital
- In a long-term care facility
- Has orders from a physician requiring skilled medical services
- Is homebound
Important Note
You must have a referral. This means that if you are enrolled in a managed care plan, the care must be authorized either by your primary care physician or by Mobile Doctors.
